Case management is the core of service delivery at the Daily Planet. The Planet provides immediate and on-going case management to adults in the Greater Richmond area who are newly homeless, as well as to those who experience multiple and complex barriers to resolving homelessness. We help those with histories of mental illness, substance abuse, incarceration, or chronic or acute medical problems-- problems that serve as obstacles to accessing sustainable income or attaining and maintaining adequate housing. Daily Planet's Case Management Services utilizes a comprehensive and integrative approach to help individuals access resources to have stable income and safe, sustainable housing. Case management helps people connect with: -Medical, Dental, and Eye Care Services -Mental Health Treatment, -Substance Abuse Services -Project STRIVE employment program -Benefits acquisition -Income management -Housing Clients who participate in Daily Planet services are also given the opportunity to utilize the laundry and shower services, the clothing closet and mail pick-up. Through case management, the Daily Planet has developed a substance abuse services component called Collaborative Treatment Services. This program assists chemically dependent homeless individuals break the cycle of homelessness by facilitating immediate access to 90 days of residential treatment at Rubicon, Inc., and providing comprehensive case management to ensure that participants access employment, housing and after-care treatment. Daily Planet Substance Abuse Services also offers assessment, referrals, and individual and group counseling. Groups are held at the Daily Planet Health Care Clinic daily: Monday RELAPSE PREVENTION 9:00AM -- 10:00 AM Tuesday EDUCATION/ EARLY RECOVERY 2:00PM - 3:00PM Wednesday CO-OCCURRING DISORDERS 8:30AM - 10:00 AM GROWTH AND RECOVERY 1:00PM -2:00PM Thursday EDUCATION/ EARLY RECOVERY 1:00 PM - 2:00PM Friday RELAPSE PREVENTION 3:00PM - 4:00PM
517 W. Grace Street, Richmond, VA Monday- Friday, 8:00 am - 4:30 pm. Intakes for case management, substance abuse treatment and mental health services are provided on a walk-in basis through the Side Door entrance. |
